Claiborne-Hughes Health Center is a locally owned non-profit health and rehabilitation center. Our facility is conveniently located in the heart of downtown Franklin, TN just off Columbia Avenue. We are located in the vicinity of Franklin's historic battlefield district and we also embrace our historic roots. If you are seeking a full service, Medicare and Medicaid Approved health and rehabilitation facility, come and visit our facility to discover why we are your hometown health and rehabilitation center.
Our Residents are our best spokespersons. We have many Residents who have had repeat stays with us. They know that the Rehabilitation staff will assist them in accomplishing their goal to become independent again and often to return home. Rehabilitation services are provided in a friendly Resident-centered manner. "The therapists get to know us as people, they find out what is important for us to do, what we want to achieve." said one recent Resident. Mrs. Joan Cameron, on a recent visit back to the facility after her return home, stated "When I came to this facility I had been prepared never to walk again, now I am home, walking anywhere I want to go and washing dishes at my sink. It makes me cry with happiness when I think of what I have achieved." Still another said, "They recognized I was depressed and that it was keeping me from achieving my goal. They worked hard with my doctor to come up with a plan that helped me to ultimately meet my goal to go home with just a little help."
We know that many people who come to the facility may not be able to return home. When the plan includes a long term stay, we work hard to establish a homelike experience for the Resident. Claiborne-Hughes welcomes the visits of small pets and the involvement of friends in family in their daily routine and care decisions. We encourage the continuation of important hobbies and interests. We mark important events, holidays and accomplishments with celebrations. Many of our Residents have bird feeders or plants just like they had at home. Common spaces in the building include birds, plants, familiar pictures and nostalgic items which encourage a feeling of belonging.
One of the most enjoyable aspect of our facility is the quality and quantity of outside space available for Resident and family use. The facility is located on three acres of wooded and landscaped grounds. There are three patios and a courtyard space appointed with picnic tables and outside seating. One of the courtyard patios is covered. The grounds have a therapy walking trail equipped with steps, benches and a bridge. Some of the landscaping has been provided by families to provide gardens like those at home.
The large shade trees provide an outside space available for leisurely strolls and picnics under the trees. When the weather is accomodating, outside activities allow the Residents an opportunity to enjoy the changing seasons. The grounds are prepared to maximize a Resident's free access to outside spaces without compromising their safety or security. There is an enclosed courtyard which allows Resident with cognitive deficits to go outside with staff members. Many Residents utilize outside areas for their own daily exercise activities and they can be seen walking the therapy trail independently or with their walkers, canes or wheelchairs.
Our lovely gazebo is one of many of the community service projects conducted over the years at Claiborne-Hughes Health Center. The gazebo is a favorite gathering spot for family visits in the warmer months and is also a location where Residents go to chat and watch all the "happenings" around the grounds. It provides a quiet spot to enjoy the birds and flowers in the shade. Our charming and historic building and ground provides just one more reason you should consider Claiborne-Hughes Health Center for you next healthcare stay.
